緩和ケアにおけるデジタルテクノロジーを活用した患者・家族への非薬物的アプローチ

[要旨] 近年,デジタルテクノロジーの進展は,医療界においても新たな可能性をもたらしており,緩和ケア領域も例外ではない. 本総説は,緩和ケアにおける非薬物的アプローチとしてのデジタルテクノロジーの活用を包括的に概観することを目的とする. 具体的には,①スマートフォンアプリ,②extended reality (XR),③electronic patient-reported outcome (ePRO),④artificial intelligence (AI) に注目し,主に国内の臨床応用および最新の研究動向を整理する. さらに,これらのテクノロジー導入に伴う患者・家族の体験や,倫理的・法的・社会的課題についても横断的に考察し,デジタルテクノロジーと人間的ケアの融合に向けた将来展望を提示する.

がんサポーター養成講座受講者の特徴と講座受講によるがんの知識・意識・認識の変化

[要旨] がん患者会「はまなすの会」は,正しいがんの知識を持った市民を地域で増やすことを目的に,がんサポーター養成講座(以下,養成講座)を開催している.本研究では,養成講座受講前後のがんに関する【知識】【意識】【認識】の変化について質問紙を用いて調査した.
 養成講座は2024年6月30日,7月28日,8月25日の計3回開催された.養成講座初日の受講前と3回目の受講後に,無記名自記方式の質問紙を配布した.質問項目は性別,年齢,居住地域,職業,養成講座を知ったきっかけ,がんに関連する20項目とし,医療関係者を除いた79名,61名を解析対象とした.
 受講者の大半は女性で主婦が多く,50~70代が多かった.がんに関する【知識】【意識】【認識】は大多数の項目で向上がみられ,抗がん剤の副作用を我慢する必要はないという知識や,「グリーフケア」という言葉の認知度が大きく向上した.本養成講座によりがんの情報を普及啓発できたと考えられた.

医学科生における医療用麻薬への認識に対する認定薬剤師による講義の教育効果

[要旨]医療用麻薬に対する医学科生の認識を評価するため,緩和薬物療法認定薬剤師による講義前後で,医学科4~5年次生108名を対象にアンケート調査を実施した.講義後は「耐性」「副作用」「最終手段」などの誤解が有意に改善した.また自由記述からも理解の深化や説明責任への自覚が示された.以上より,卒前医学教育における医療用麻薬教育の一事例として,認定薬剤師による講義が一定の教育的意義を持つ可能性が示唆された.

Exploratory Study of Prognostic Predictors for Patients with Terminal Lung Cancer

[Abstract]【 Objective】In palliative medicine, predicting the prognosis of terminal cancer patients is crucial.
Previous studies have reported associations between prognosis and cachexia-associated protein levels in the
palliative cancer care setting. However, several reports were stratified by cancer type. This study aimed to
determine the prognostic factors in terminal lung cancer. 【Patients and Methods】Of the 756 terminal cancer
patients who were hospitalized for palliative care and who died during their hospital stay over a three-year
period from April 2018 to March 2021, 95 patients with histologically confirmed primary lung cancer were
included in this study. In the subgroup analysis, the cases were further classified as adenocarcinoma (ADC),
squamous cell carcinoma (SCC), and small cell carcinoma (SCLC). At admission, blood tests were performed to
measure the levels of serum albumin (Alb; g/dL), transthyretin (TTR; mg/dL), and C-reactive protein (CRP; mg/
dL), and their association with prognosis was assessed.【 Results】The multivariate analysis identified CRP
(hazard ratio [HR] 1.0403, 95% confidence interval [CI] 1.0064-1.0734; p = 0.0205) as an independent
prognostic factor, with a 4-week survival cutoff value of 2.5. There was a difference in 4-week survival rate
between those with CRP ≥ 2.5 and those with CRP < 2.5. In ADC, serum TTR (HR 0.9273, 95% CI 0.8777- 0.9754; p = 0.0050) was considered an independent prognostic factor, with a 4-week survival cutoff of 13.0. There was a difference in 4-week survival rate between those with TTR ≥ 13 and those with TTR <13. 【Conclusion】Serum CRP was considered an independent prognostic factor in all primary terminal lung cancers, whereas serum TTR was considered an independent prognostic factor in ADC alone. There was a difference in 4-week survival rate between those with CRP ≥ 2.5 and those with CRP < 2.5 in all primary terminal lung cancers. Furthermore, there was a difference in 4-week survival rate between those with TTR ≥ 13 and those with TTR <13 in ADC alone.
